Q: How do I get a “Celebrate Nursing” license plate?
A: Plate requests are made on the Special Order Plate Application (form M-22). Download, fill out and mail your application with check for appropriate fee to the address listed on your application.

Q: Do I have to go to the DMV?
A: All applications should be filled out and mailed with check for appropriate fee to the DMV address listed on your application. Your “Celebrate Nursing” license plates will be mailed to you. You can drop off your old plates at your local DMV.

Q: What if I want to keep my old license plate number?
A: You can maintain your current marker plate number and have it re-made with the Celebrate Nursing background. See the DMV website for exact prices.

Q: Can I get a “Calebrate Nursing” and a vanity plate?
A: Yes, you can. There is an additional cost for the vanity plate but it is equal to the price of a standard vanity plate. See the DMV website for exact prices.

Q: How do I choose my vanity plate?
A: The DMV provides instructions on choosing your vanity plate. See the Special Order Plate Application for details.

Q: Can I change my registration type when I get my “Celebrate Nursing” license plate?
A: Unfortunately, no. You are required to maintain you current registration type. See the DMV website for more details.

Q: Can I get a “Celebrate Nursing” license plate for a car registered in another state with temporary Connecticut plates?
A: Unfortunately, no. Your vehicle must be registered in Connecticut before applying for a “Celebrate Nursing” license plate.  See the DMV website for more details.

Q: Where is my money going?
A: CNF recieves 75% of the proceeds from each “Celebrate Nursing” license plate purchased. Those funds directly support our Foundation Nursing Scholarship.
